Fall Sports Physical Deadline Information

Parents/Student-Athletes:

All students must submit all needed athletic documentation in order to participate in ANY Conditioning, Practices, or organized school athletic events BEFORE PARTICIPATING.

The Last Day to submit physicals for FALL SPORTS is the first day of Tryouts (August 21st).  

Directions:

Parents/Students must create a DragonFly Account (https://www.dragonflymax.com/).

Parents/Students must request to join Lowes Grove Middle School Athletics.

Once access is granted parents must upload all documentations to the student’s profile: Physicals, Health Insurance Portability & Accountability Act, Concussion Documentation, Participation Documentation

Finally, Students must submit a hard copy of their Physicals & Health Insurance Portability & Accountability Act forms to the Athletic Director BEFORE PARTICIPATING IN ANY ATHLETIC EVENT.

STUDENTS WILL NOT BE ELIGIBLE TO PARTICIPATE IN FALL SPORTS UNITL ALL DOCUMENTATION IS RECEIVED AND UPLOADED INTO DRAGONFLY WEBSITE, PER DPS ATHLETICS POLICY.
